2 September 2026 Punjab Khabarnama Bureau  :  A severe case of H1N1 influenza in Delhi has highlighted how an undiagnosed health condition such as type 2 diabetes can complicate recovery from a serious viral infection.

A 41-year-old woman, who was unaware that she had diabetes, developed severe H1N1 symptoms and experienced dangerously low oxygen levels. Doctors discovered her diabetes only during the hospital evaluation, adding another challenge to her treatment and recovery.

Fever Quickly Turned Into Severe Respiratory Illness

The woman, identified as Megha Garg, initially developed fever on August 12. Her symptoms progressed to cough, cold, intense body pain and persistent chills.

By the fourth day, her oxygen saturation had fallen to 80%. She also began coughing up blood, a serious symptom that prompted further medical evaluation.

She was admitted to Apollo Hospital on August 15 and diagnosed with H1N1 influenza. Because of her low oxygen levels, she required oxygen therapy and non-invasive ventilation and was kept under close observation.

Diabetes Was Discovered During Testing

During the evaluation, doctors found that Garg’s HbA1c level was in the diabetic range, indicating that she had been living with persistently elevated blood sugar for some time without knowing it.

Dr Nikhil Modi, senior consultant in respiratory medicine at Indraprastha Apollo Hospitals, said the patient had undetected diabetes and a very high HbA1c level.

HbA1c reflects average blood glucose over roughly the previous two to three months, so the finding suggested that the diabetes predated her H1N1 infection rather than simply being a temporary effect of the illness.

Why Diabetes Can Make Flu More Difficult

Diabetes can affect several parts of the body’s immune response.

High blood sugar may impair the ability of white blood cells to respond effectively to pathogens, while chronic metabolic stress can contribute to abnormal inflammation.

According to Dr Modi, this can create a difficult situation in which the body’s ability to clear an infection is impaired even as inflammatory responses remain excessive.

Patients with conditions such as diabetes, hypertension and cancer may therefore take longer to stabilise and can have a higher risk of deterioration during severe influenza.

A Potentially Dangerous Cycle

Severe influenza can place additional stress on the body, while uncontrolled blood sugar can make it harder to fight infection.

Experimental evidence also suggests that elevated glucose may promote influenza infection and replication in respiratory epithelial cells, although the precise contribution of high blood sugar to influenza severity in humans remains an area of research.

This means doctors may need to manage both the respiratory infection and blood glucose levels simultaneously.

Treatment Included Antiviral and Diabetes Management

Garg was treated with the antiviral oseltamivir, alongside respiratory support and treatment to bring her blood sugar under control.

She remained in hospital for nearly a week and was discharged on August 22. Doctors prescribed insulin and diabetes medication in low doses, with longer-term treatment to be adjusted as her acute illness settled.

The Case Raises a Bigger Question About Undiagnosed Diabetes

Garg’s experience also highlights the fact that type 2 diabetes can develop gradually without obvious symptoms.

People may not realise they have elevated blood sugar until they undergo testing for another health problem.

Screening may be particularly important for people with risk factors such as excess weight, a family history of diabetes, high blood pressure, abnormal cholesterol, a sedentary lifestyle or a previous history of elevated blood sugar.

Does Everyone Need Regular Blood Sugar Testing?

The case does not mean that every healthy person needs to repeatedly check blood sugar at home.

However, people who have never been screened and have significant diabetes risk factors should discuss testing with a doctor rather than waiting for a serious infection to reveal the problem.

A blood glucose test can provide a snapshot of current sugar levels, while an HbA1c test helps identify persistently elevated blood glucose over several months.

When Can H1N1 Become Dangerous?

Most people with influenza recover without hospitalisation. The risk increases when the infection progresses to complications such as pneumonia or respiratory failure.

Urgent medical assessment is warranted when a person develops:

  • Difficulty breathing or severe breathlessness
  • Persistently low oxygen saturation
  • Chest pain or pressure
  • Bluish or grey lips or face
  • Confusion or unusual drowsiness
  • Severe weakness or dehydration
  • Coughing up blood
  • Fever or cough that improves and then suddenly worsens

People with underlying conditions, including diabetes and chronic heart or lung disease, may require earlier medical attention when they develop influenza-like symptoms.

The Key Lesson From the Case

The case illustrates that a serious infection may sometimes reveal an underlying health problem that had previously gone unnoticed.

For patients, the message is not to panic but to understand their health risks, seek appropriate medical care when flu symptoms become severe and discuss diabetes screening when risk factors are present.

For doctors, identifying and managing conditions such as diabetes alongside H1N1 can be an important part of preventing further deterioration.
